How Support Coordination Can Help NDIS Participants

The aim of support coordination is to help simplify your understanding of the NDIS.

At Care Support Network, we realise that in order to help you achieve measurable outcomes through your NDIS plan, you first need to be able to understand it.

Our dedicated team of coordinators ensure that they have equipped you with enough information to help you build an understanding of your stated supports. We will also guide you through the necessary steps required to help you make informed decisions that allow you to confidently engage with other service providers of your choice.

Care Support Network will work with you (and your existing services) to ensure that you are able to access, build and maintain meaningful relationships at home and in your local community.  We will also work with you to strengthen your capacity in the management of your own supports.

The NDIS will outline the type of support you require in your plan. These supports may be:

Support Connection – Short-term support to implement your NDIS plan and identify service providers
Support Coordination – Longer-term support to assist you to navigate various type of services and supports you will need from a range of providers.
Specialist Support Coordination – Specialist support coordination where you have more complex needs and you require longer-term support.
