What is a Home Care Package (HCP)?

A Home Care Package is a government subsidy provided to help you live at home as long as you want. The funds are provided to an approved home care provider, who coordinates a package of home and health care and case management services to meet your needs.

Home Care Packages are provided under a consumer directed care model, which means that the choice of how to spend your package funds is in your hands. You and your approved provider become partners in developing and managing the care you need.
  

Levels of Care & Support (HCP)

​​There are four different levels of Home Care Package each designed to support people with different care needs. Each level of Home Care Package provides a different funding amount.
​​
* Home Care Level 1 – a package to support people with basic care needs.
* Home Care Level 2 – a package to support people with low level care needs.
* Home Care Level 3 – a package to support people with intermediate care needs.
* Home Care Level 4 – a package to support people with high care needs. 

How Do I Access A Home Care Package?

First you need to contact and register with My Aged Care by phoning 1800 200 422 . It’s a good idea to visit their website www.myagedcare.gov.au to learn more about what you can expect and information you will be required to provide.

Following your conversation with My Aged Care, you may be contacted by the My Aged Care Assessment Team (ACAT) to organise for a trained assessor to visit you in your home. They will assess your care needs and eligibility for services, and work with you to develop a support plan which addresses your needs, goals and preferences.

From there you will be given more information on costs, financial assessments and service options.

When you have been approved for HCP funding, Care Support Network can then assist you to coordinate and manage your package to achieve your support goals.

After your assessment, you will then need to choose a service provider who manages Home Care Packages (Level 1 -> 4) such as Care Support Network (CSN).

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Programme provides entry-level home support for older people who need assistance to keep living independently at home within their community.

It is assessed by the Regional Assessment Service, and offers a more limited set of services than is available through a home care package. It includes the Commonwealth Home and Community Care (HACC) Program.
